Seizure Action Plan
* Students who have a current diagnosis of seizures, should have a Management Plan for Seizures on file with the School Health Office.
* If the Management Plan indicates that medication is needed, medication should be provided to the Health Office with written orders from the provider (see "Medications to be Given at School" tab of this website).
* A blank copy of all necessary forms can be picked up in person from the school nurse or download from the link below.
