Food Allergy Information
* Students with food allergies must have a valid Emergency Allergy Action Plan on file for each school year. This form must be completed by a medical provider and signed by the medical provider and parent.
* Food allergies will not be honored by the cafeteria unless an Action Plan is on file with the school's Health Office.
* If an Action Plan indicates that the child should have a rescue medication available at the school, this plan must be provided to the school with a valid doctor's order for the medication and the medication. If no medication is required for the allergy, the parent/provider should just write "No medication required" on the plan.
* A blank copy of all necessary forms can be downloaded from the link below or picked up from the school nurse.
