Asthma Management Plan
* Students who have a current diagnosis of asthma should have a Management Plan for Asthmatic Student on file with the School Health Office.
* If the Management Plan indicates that a rescue inhaler is needed, medication should be provided to the Health Office with written orders from the provider (see "Medications to be Given at School" tab of this website).
* If the guardians and provider feel that the student is capable of carrying and self-administering his/her inhaler, this must be indicated on the Management Plan and medication order. This must be cleared through the School Nurse or the child will be in violation of the Code of Conduct for carrying medications at school.
* A blank copy of all necessary forms can be picked up in person from the school nurse or download from the link below.
